Support disabling of lobby ratings in the GUI

Description
The current lobby GUI assumes ratings are enabled. But there have been times where the rating bot was removed and for any reason, we might temporarily or not disable ratings. The rating bot could also happen to be unintentionally offline (then the rating won't be affected when finishing a rated game).
If the rating service is unavailable, the leaderboard button and the user profile lookup button should become disabled and the rating column in the player- and gameslist should be hidden.
The service can be assumed unavailable until a rating was received the first time.
