Formation is forgotten

When setting a formation for an army it will randomly (seemingly) lose it when in a fight.

Can you check which formation settings you use for:

• default formation
• formation override

Can you describe the problem a little bit more?

It's 'normal' that the units lose their formation if you've enabled the 'walk only' formation override option (which is the default).

The formation override is just "walk/patrol" or "none", so I'm not sure we're talking of the same thing.
